- Multinational Company/ Public Listed Companies/Subsidiaries Companies:
- Latest 1 month salary slip;or
- Latest EPF statement; or
- Latest EA Form; or
- Latest income tax return (Form B/BE with tax receipt)
- Other Companies / Non-Multinational Company:
- Latest EPF statement printed on AFFIN kiosk; or
- Latest income tax return (Form B/BE with tax receipt); or
- Latest 1 month salary slip with latest EPF statement or latest bank statement showing 3 months salary crediting
- Government/Semi Government office:
- Latest 1 month salary slip
