- Photocopy of MyKad (both sides)
- Latest month salary slip for fixed Income earner or latest 3 month salary slip for Variable Income Earner OR
- Latest EPF statement with minimum 6 months contribution.
Other Companies
- Photocopy of MyKad (both sides)

Self Employed (Malaysian):
